Presbyterian Homes Westminster is seeking highly qualified Nursing professionals to join our staff. ABOUT THE OPPORTUNITY:
LPN provides care for residents of rehab, long-term care and/or Personal Care facility as prescribed by a physician or nursing leadership.
EDUCATION AND REQUIREMENTS:
*Active license to practice as LPN/LVN and in good standing in the State of Kentucky.
*Graduate from approved School of Nursing. Geriatric Nursing experience preferred
DUTIES AND RESPONSIBILITIES:
•Perform day-to-day functions in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
•Complete and file required record keeping forms/charts upon the resident’s admission, transfer, and/or discharge.
•Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
•Receive telephone orders from physicians and record on the Physicians’ Order Form and enter into computer system as needed.
•Transcribe physician’s orders to resident charts, medication administration records, and treatment/care plans, as required.
•Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care.
•Prepare and administer medications as ordered by the physician.
•Ensure that narcotic records are accurate for your shift.
•Review care plans daily to ensure that appropriate care is being rendered.
•Make any changes that need to be made on the care plan and communicate the changes to the desk nurse.
•Ensure that your nurses’ notes reflect that the care plan is being followed when administering nursing care or treatment.
•Perform other duties as assigned
•Must have a valid LPN, license in good standing to practice in the state of KY.
•Must be able to bend, stoop, push, pull and lift using good body mechanics in order to avoid injury to self or others.
•Must be able to read, write, comprehend and speak English.
•Must have excellent customer service skills, professionalism and compassion in dealing with residents, staff and visitors.
•Must be detail-oriented.
•Must be knowledgeable of Resident's Rights and adhere to such.
•Must be dependable - at work and on time for each scheduled shift.
