The Home Manager provides/oversees that continual training is present in the assessment, development, implementation, and/or management of assisting individuals to attain their own personal outcomes.
Personal outcomes for individuals may be achieved by opportunities being present in the areas of self-identity, autonomy, affiliation, attainment, safeguards, rights and health wellness. Trains, supervises and evaluates program staff ensuring evidence of personal outcomes for consumers.

Education requirementsBachelor’s Degree in a Human Services field preferred

Experience requirementsEither a Bachelor’s Degree in a Human Services field and two years directly related experience or five years of directly related experience in the Human Services field according to Missouri Department of Mental Health standards

QualificationsOther requirements: Use of a personal motor vehicle, valid driver’s license, adequate auto insurance as defined by state law. Acceptable police, DFS, and DMV record checks.

ResponsibilitiesA. Ensure that staff working at the site provide care and support to individuals receiving services in accordance with all legal regulations, policies, procedures, and training
B. Ensure all Waiver guidelines and regulations are met or exceeded on assigned site.
C. Position is salaried. Must work 40 hours per week. 32 hours on schedule for the home that will typically be opposite of the Lead Staff and 8 hours of Administrative time. Provide 24-hour on-call support to assigned work site, and respond, within one hour, to all phone calls. On call two weekends a month from 5 p.m. Friday - 9 a.m. Monday (shared with Lead Staff). Sends out group texts on Friday and Monday to communicate to all team members who is on call.
D. Check and read Sunnyhill emails on a daily basis. Respond within 48 hours and provide follow-up as appropriate
E. Complete 90 day evaluations and Annual Appraisals for all staff.
F. Train, supervise, and document the supervision of all staff at assigned work site. Monitor all expirations of training requirements and ensure that all required agency training for all assigned staff is completed in a timely manner. Ensure that all Lead staff and DSP’s review, and sign that they have read and understood all ISP’s and BSP’s.
G. Attend all monthly Nursing visits and review and sign monthly nursing summaries.
H. Create staff schedules, ensuring appropriate staffing in accordance with contractual staffing pattern and budgets, and communicate any staffing needs to Waiver Management. Supervise In-home orientations for all new staff members.
I. Coordinate all aspects of filling direct care shifts as needed, without utilizing overtime hours.
J. Work direct support as needed to offset overtime and/or provide coverage due to unexpected schedule changes or to meet the needs of the individuals receiving services
K. Complete and submit Variance Report data for all consumers at the site
L. Discipline staff when necessary according to their job description, employee handbook and/or Sunnyhill policies and procedures. Responsible for composing discipline, scheduling employee, and serving discipline within 3 business days
M. Review and submit LS, DSP, and HM mileage with new Concur/Company Mileage by the 5th of the following month
N. Review all daily log notes written for assigned site and ensure that all notes are written according to Waiver regulations and guidelines
O. Ensure that daily log notes provide a complete record of staffing support that was provided to each individual receiving services
P. Document all support provided by the HM to the site, clients, and staff on HM log notes. Each month’s log notes must be submitted to Waiver management by the 15th of the following month
Q. Track all client progress regarding their personal outcomes throughout the plan year and write monthly outcome reports documenting such progress. Each monthly outcome report must be submitted in taskmaster and placed the books by the 15th of the following month.
